My true and correct name should be Ernesto Toriado ORCINE, Jr. which I used and still using, but in my certificate of live birth, it appears to be Ernesto Toriado ORCENE, Jr. Should this be covered by RA 9048? If not, what legal action should I do? Please advise. Thank you!

How much will I pay to correct my name from Maria Andrade Liongson to Maria Theresa Andrade Liongson? What are the requirements/documents needed? How long it will take for to grant my request? hi sir/mam:

I’m applying for a passport, and i was married two years ago. The problem is, I’ve only noticed till now that my mother’s name in my marriage certificate(both NSO and municipal registrars office) was AURORA J. VENTURA instead of MA. AURORA S.J. VENTURA. What will i do to correct this? Is it still okey, to use this for my passport application? Please advice. Thanks.

hi attyatwork!

Gusto ko lang po sana itanong kung paano papalitan yung surname ng anak ko? kami pong parents ay di pa kasal pero ina-acknowledge nung tatay yung aming anak kaya nagamit nya yung apelyido ng tatay. Ngayon kasi kailangan maibalik sa apelyido ko for iimigration purpose, before po kasi ininterview ang aking asawa para sa petition po ng pamilya nila papuntang canada, hindi nya naideclare na may anak sya kasi nung time na yun 3 months palang akong pregnant. 8 months na yung baby namin nung nakaalis sila, at ngayon para maasikaso ang papers naming mag-ina kailangan daw muna maibalik sa apelyido ko yung baby.

hello,
august 12,1959 ang nakasulat sa nso at sa local registrar na birth of
date, bago ko lang nalaman ito ng kumuha ako sa nso ng bc. me nag
-advice na sundin ko na lang ang nso record para walang masyadong
gastos, ang tanong ko paano ang iba kong papeles babaguhin din po
ba ang birth of date ko at ang mga birth certificate ng mga anak ko na
may august 22,1959 (ito po ang gamit kong birth of date ko mula noon
hanggang ngayon) ano po ba processo nito?
